Why is my form not working?

I haven't changed this form / logic etc. since last year.  the totals are not populating in the total check/credit card fields.  Can you help?

I have just checked your form calculations and they seem to have been properly configured, upon testing your form, I can see the calculations work correctly, example:

If the problem persists, please let us know if it happens only while making certain selections and the browser you're using. 

I actually figured out the problem right after sending this.  I had extra + signs in the calculation field that were affecting things.

How do I find submitted support tickets after I send if I want to update?
