PDF Editor>>Import Fillable PDF: my fillable PDF form is not being imported.

I have a fillable PDF that I created in Adobe Acrobat. Each time I try to upload it using this link https://www.jotform.com/fillable-pdf-form-creator/ it just has 3 dots that load forever and never do anything. Please help. 

For clarification, I make it to the screen that says "we are now importing your PDF" then it goes to the 3 loading dots and never changes. I have waited for hours, refreshed, tried again. No change.
