Transferring an existing form to another account

I'm creating a registration form for our school and I started it on my JotForm account but need to transfer it to the school's account. Can that be done?

Also, how do a make if...then questions in my form? If you register a child then i want it to pop up a question "what size t'shirt?" and a scrolling number.
