Why is my acct suspended

    Why is my account suspended and how do I fix it? I have emailed you guys twice with no response

    Upon review of your account it was found form:

    http://www.jotform.us/form/90617984867173 is collecting social security numbers and some other of your forms are offering loans which is only allowed to financial institutions.

    Please furnish proof of affiliation in order for us to be able to reactivate your account.
