Print button not working on the form embedded on our web page

Hi there

We have added the form to our website but the print button is not working on the website, the link is here: https://gwcregaliahire.nz/hire/

Have tried so many things like clearing cache of wordpress, clearing cache of form, removing and adding the print button again etc...
