Conditional Logic Not Working

Hi there,

The conditional logic on this form was working and now it's not and I don't know why.

Essentially, if New Order Type is "New Lawn Signs" and My Compass Office equals Weston or Wellesley, they should get an option for Delivery under the New Lawn Sign section. If they are in any of the other five offices, they should have an Installation or Storage option. 

The Weston/Wellesley logic is working but if they choose any other office, the Installation/Storage option does not show up.
