Form not submitting

My simple form is not submitting, getting stuck at "Please wait..." after clicking the submit button.

Can you look into this? The form was supposed to go live yesterday, and no one has been able to fill it out.

I test submitted your form and I was able to see the problem as well.

Upon reviewing the form in the Form Builder, I found that the issue was being caused by 2 unused widgets added to the form https://www.jotform.com/build/90854037135153

To fix, please delete/remove those 2 unused widgets and Save the form.

Submission of the form should now succeed after.
