I am experiencing bizarre pdfs

    I created a Road Opening Permit Application cloned from a Curb, Sidewalk & Driveway Application. When someone submits a Road Opening Application, it includes a pdf version (which we print out). The pdf version has the title of "Curb, Sidewalk & Driveway Application". It only includes fields that were in the Curb, Sidewalk & Driveway Application and leaves out many other fields in the original Road Opening Permit Application. Please help.

    Have you applied a PDF Template to your new form? Please create a new PDF Document to resolve this issue.
