How can have a full name with middle name on the same line?

I sent a form in earlier today on "how can I add an additional line ex. Name Middle Last I need for this to be on the same line

I received back a reply going Vertical I need it going horizontal  Ex.

You seem to have figured that out already. You have a full name with middle name enabled, and they are on the same line in your current form which
