Can I upload a form I already have that is in a word document?

Unfortunately, it is not possible to import directly a word document form to JotForm.

If you have a word document, you can still import it by first converting the word document to an HTML file, you can follow this guide: http://www.wikihow.com/Convert-a-Word-Document-to-HTML. 

After that, upload the HTML file on a web hosting site or a server so that it can act as a live website. You can then follow this guide on how to import HTML forms in a website: http://www.jotform.com/help/28-How-to-Clone-an-Embedded-Form-from-a-Webpage.
