My Form has blue "?" boxes in each of the submit fields

    How do I remove the blue boxes from the input fields?

    Why are they there?

    The images used in the form are no longer available from where they were hosted so they are now showing as broken images:

    Either the image URL's could be updated to other images or those blocks of code removed to remove the images entirely.
