My customers are not receiving a confirmation and I am not receiving their form

    my customers are submitting forms but not receiving a confirmation or receiving confirmation w/ incorrect info - and i am not receiving their form at 

    please see email from client: 


    I filled out the on-line form at the link you sent me but the confirmation email and pdf that I received back is totally wrong (see attached). Most of the information is missing and the colors and locations are not reported correctly.

    Also, it would be helpful if you can provide us with the screenshot of the issue or the customer information that submitted to investigate further.
