URGENT! Form is not responding and is too slow

Hi, we are having some major issues our new form. It is almost not accessible and is too slow to respond. Please take a look to see what is going on. We want to get this squared out this weekend against Monday when we plan on officially sending it out.

The form load time is a bit slow due to a large background image of your form (20.8 MB). I would like to suggest optimizing the image for web, then adding the optimized image in your form. It should improve the responsiveness of the form.
