Account recovery from being flagged as phishing attempt

    Dear Jotform support,

    My account "pakaeo" and my IP address have been banned due to a possible phishing attempt.

    It has been about 48 hours since I filled in the form, but have not heard anything back from you. 

    I will need to use the forms within the next 24 hours, and I am awaiting your response. Should you require further information that I have not provided, please let me know. I am more than happy to give you further explanation.

    Upon review of your account it was found form id:http://www.jotform.me/form/91642483600454 was collecting ID information. We will need you to remove the collection or add the two mandatory widgets for compliance:

    1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

    We have reactivated your account so you can make the appropriate modifications.

    Accounts are subject to screening thereafter. Please make the changes to comply, if the form is still found without the widgets, the account will be subject for suspension again.

    I have now removed the question regarding ID number on all forms.

    Could you also allow access from my IP address, please?

    Hi,

    My account is active but I have to use a VPN service every time I visit Jotform.

    It would make my life much easier if I could access the site directly.

    We have now whitelisted the IP address for your account. Please check now and see if you are able to login normally. If the issue still persists, please provide us with the public IP address so that we can whitelist.

    I still got access denied, and my public IP is 159.192.216.89

    Hi,

    I have gone ahead and whitelisted that IP Address. Please let me know if the issue persists.
