How can I create a form and link it to Trello?

I am trying to figure out how to use a form that I created and link it in Trello. What the goal is, would be to have a new Trello Card created each time the form is used since it would have new information.

Currently, the form keeps updating and making changes to the same single card. 

Is there an actual way to do this please?

We do have a direct integration with Trello.

Go to your Settings>Integrations>and find Trello in the search bar

Then login to your Trello account and you can select the action that you want.
