Allow users to upload Avatar/Image

How can I get a user to upload their avatar as a choice in my JotForm. This feature will allow me to see the users if they decide to submit it on the form.

You can make use of the Image Upload Preview widget to achieve this requirement. In this widget, a user can upload image of their choice to be their Avatar.
