Show or hide condition is not working on my form

I am trying to get the Name2 field to show the combination of 2 other fields when a user selects the Week 2 option. The email results only show Name, not Name + Week 2.

My understanding is you want to show the Name2 field whenever a user selects the Week 2 option. If yes, please add a show and hide condition to your form and follow the screenshot setup below:
