My form has been disabled

    My form has been disabled and I can’t login to my account. Says it’s banned bc of a possible phishing attempt. I don’t even know what that is. Https://form.jotform.com/91684954617168

    The form is created with a long text entry field that asked for CC information:

    This is the reason why you are suspended. We can reactivate your account, but you have to agree with our terms and we will have to delete the field(s).
