I would like to set up a form with a list of questions that have multiple choices

    Hello,

    I'd like to set up a form with a list of questions that have multiple choices.


    In my attached screen shot, I'm using check boxes, I'd prefer to have "YES", "NO", "N/A", options for each question in the same format as the check boxes.


    Is this possible?

    Use Multiple Choice field to add a checkbox. You can edit the options within the editor or in the checkbox properties.

    If you want another checkbox field under each of the checkbox options, there's no direct solution for that. A workaround is to have separate checkboxes. One parent checkbox and one checkbox under it. You can then customize their options accordingly.

    The look can be further customized by injecting custom CSS codes to your form.

    It’s not exactly the format I was looking for but it will work, I was hoping for something more compact so that I could just fill it out quickly with having to scroll through all of the questions while looking at it on my phone.

    Please note that you can display all the checkbox options in one line and multiple columns. Please check the screenshot below:

    You can customize it further by injecting custom css code in form.

    You may also consider using our Input Table for grouping multiple questions into one field.
