Request to build a form that includes products and quantities

I am interested in having you guys build a form for me i am currently using the product ordering template which is great but I'm having issues when it comes to products and quantities i am attaching my price sheet so you can see for yourself. i need this done asap and can pay right away please let me know if you can help :)
