When I attempt to log in there is a message that pops up and says suspended. I have never seen this before so I am unsure of what this means. What does this mean?

Your account has been suspended because of this form: https://www.jotform.com/build/91677986997186 and the reason is that you were collecting credit card information:
In order for us to restore your account, those fields have to be removed since collecting credit card information via plain text is against our Terms of use.
If you want to collect payments, you have to use one of our payment integrations: https://www.jotform.com/help/323-Mastering-Payment-Form-Integrations-with-JotForm

Since the CC fields are removed from your form, I am activating your account. Please use our payment integrations instead and avoid such sensitive data collections through regular form fields.

I understand now the reason why my account was suspended. I want to delete the form to collect credit card information and restore my account. Please let me know the steps to follow. And please let customer know next time the reason why their account was suspended. Wating for an answer..

I removed the guilty fields and restored your account.
Please avoid collecting credit card information with plain text to avoid further suspensions.
