I am trying to create a form which requests 3 years' address history

    Hello,

    I am trying to create a form which requests 3 years' address history. It would need to request current address (obviously) and from/ to dates. In the event someone enters dates which do not cover a 3 year period, then an extra address is required. And if the second address still does not cover a 3 year period then another address is required. And so on... 

    Is this possible on JotForm?

    It is possible to achieve show additional address field based on the specific date.

    1. Add an address field and two separate date fields for From and To dates. 

    2. You can then add another address field and From and To date field and make it hidden. Depending on your requirement you can also add a couple more address and From and To Date fields. 

    3. You can now use conditional logic to show fields if the From date in the current address field is not fulfilling the 3 years criteria.

    4. The same logic can be applied to the Previous address 1's From field to show additional address and date fields and so on.

    Hello, thanks for your help. I don't quite understand the 1012 bit in the value?

    I believe it's the total number of days in 3yrs. However, when I multiplied 365 x 3, the total is 1095. I'll confirm it with my colleague, but that is what I guess the 1012 was.
