How to create a header with text to the left and image to the right?

I have a problem. My forms all look too plain and not branded. FOr example https://www.imodular.com/p/sell-your-modular.html 

Can you help me create a form that has a header with text to the left and space to the right for an image in the same header area? 

I need the forms to be more convincing for my visitors.
