Empty fields are displaying on the email notification.

I have 2 fields that show up on every email even when blank and I have noted as hidden. How can I find out what's causing this?

Could you please share the URL of the form? Also, have edited the email template? Please note that if email body table is modified, 'Hide Empty Fields' and 'Update Email' options cannot work. That's because the table loses connection to form.
