All my forms are disabled!

Hey all my forms are disabled and say "form not found"

Please help.

Your account was suspended because you are collecting login information on your Kickoff USP form, which violates our Terms of Use:

Phishing. You agree that JotForm, Inc. may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

Please agree with our Terms of Use and allow us to delete all offending fields from your form so that we may re-inspect and reactivate your account.

Oh thanks, I wasn't aware. I just deleted the login information boxes on the form,  let me know if there is anything else that violates any terms!

Other than that it won't happen again, I agree to the terms, love this service and will continue to use it properly:)
