Why is my form disabled?

    Why is my form disabled? Please help. Because I really need it.

    We are sorry for the inconvenience this may have caused. I see that your account has been suspended due to the form titled 'Diamonds Mobile Legends' which is having ID fields. Please note that it is not allowed to create login forms or collect sensitive information like Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on phishing.  

    We request you to provide us with more information on the ID fields on your form and their purpose so that we can check and see if we can activate your account.

    the fields in this form are intended so that the diamond top up process is not lost. And maybe in some game voucher sellers or if you can see the codashop as a service provider for top up games such as vouchers or diamond mobile Legends it also requires an ID so that the game voucher purchase process is successful  .  and this is not taking other people's data.  can check at https://www.codashop.com/id/mobile-legends it also asks the user or buyer to enter their game ID and it is not dangerous to the buyer or user earlier.
