The product on my form are not visible for certain option

Hi,

In my form " Chaboya PTSA Membership & Donation Form" we have 4 payment types and information on different donation level.

The donation level is should be visible for all payment type. But somehow for Bank Check and cash, these donation option are not visible. Can you please help to fix it.

Both conditions related to payment field are broken:

That's why conditions are not firing.

When you fix them it should work fine, and you can do that by editing each condition and then just save.
