Login suspended

    tenho uma conta no jotform com este e-mail,  do nada não consigo fazer o acesso , diz que meu formulário foi deletado, e não consigo fazer o login.

    gostaria de uma posição.

    Your account was suspended and your forms were disabled. The forms are not deleted.

    The reason for the suspension is that the form "91682175757671 - Cadastro de Cliente Futuro Telecom" was collecting ID information which cannot be done directly as per our Terms.

    You will need to add the widgets mentioned below in your form so that your customers authorize the collection of the ID info.

    1. Terms and Conditions - https://widgets.jotform.com/widget/terms_amp%3B_condition  

    2. Signature - https://widgets.jotform.com/widget/signature 

    Please agree to add these widgets in your form and only then can we remove the suspension on your account.

    Obrigado por me responder , eu realmente não li os termos de uso.

    Por gentileza o senhor pode inserir os widgets necessário para minha conta ser desbloqueada ?

    Bom dia ,
    Sim , eu autorizo a inserção do widget mensionado.
    Também autorizo a inserção do signature.
    O importante é eu não ter minha conta bloqueada.
    Obrigado desde já e aguardo o desbloqueio.
