HOW/Why Does This Happen?

I changed my form and then did a PREVIEW.  The Preview looks fine

But when I go back to EDIT my form it looks like this??

It reverts back to things I either CHANGED or DELETED??

I have checked your form and I can confirm that it has now the same fields and format as the one shown in your first screenshot.

A cloned version of your form is also the same when viewed on the builder page.

If the issue persists on your end, it's possible that the issue you are having is related to browser cache. I would suggest that you try clearing the cache of your browser — http://www.refreshyourcache.com/en/home/
