Customisable form howto design and the CSS

    Hey there

    I am trying to design a customisable form that will let user list the below


    Occupancy is a drop down (2 selections)

    Address is text input but the user

    Value is text input by the user

    LVR is either input by the user (OR) works it out automactically by running a calculation the user cannot change and shows the result i.e loan $ (divided by) Value $) but this is not essential


    I also then want the user to be able to press the (+) button to be able to add more addresses to the form


    Is there a way to also make this user friendly on a mobile - seeing the lenght is long it might be better to work on top of each other instead going down the page


    Any support much apprciated
