Problems with Zapier - Jotform --> Wave apps integration

Hello

I setup an integration in Zapier, but I am getting errors.  

This is what I am trying to do:

(Planning and Useage Declaration Form in Jotform)

--> When this form is submitted, I would like WaveApps to create a new customer.

I have been testing this Zap and it worked on my end, example: 

The error shown on your screenshot states the customer name is missing, make sure you're mapping that field on your Zap and you're submitting data on the field you're mapping from JotForm, for example, if you will map the "Full Name" field, make sure it's required and filled.
