The data for the fields from my form is not showing up when I use zapier.

I am trying to Zapier my jot forms to a web hook. When I try to match up data from the Jot form into the web hook, is shows no data for my entries. 

I have multiple samples and for example, the first name data is empty when trying to match up with zapier and web hook. 

I provide a screen shot, as you can see... fields Name is empty as well as Email and every other question on my form. 

To isolate the issue, please remove the old Jotform connection first and create a new connection.
