Suspended - AGAIN

    Hello JotForm,

    We only talked a short while ago, in which you made us jump through lots and lots of hoops in order to have our forms with you. 

    The forms were then bought up to all your standards, including disclaimers, privacy policies, signatures etc. We explained these were for staff purposes and even reflected this very clearly on the forms. 

    On attempting to log in today, it states the account is yet again suspended. Could you kindly remove this suspension. As previously advised, we work within your terms and conditions and the law, and are a government approved ambulance service.

    This suspension seems to be related to a form in your Trash folder that was collecting Driver license info and passport via upload fields, without the use of the mandatory widgets (i.e. Terms and Conditions & Signature widgets) in the form.

    Please remove this form from the Trash folder. I've activated your account now.

    Having checked the trash folder, and the archive, I can confirm there are no items at all in here to purge. 


    I assume that you've done that for me. I've also double checked the forms and confirm they meet your conditions. If you have any other issues, if you could kindly advice me to avoid future suspension.

    You may refer to our Terms-of-Use and ensure that your forms comply with the said terms to prevent future account suspensions.
