Field is not showing up based on my condition

I am trying to create a condition on a single item question where one of the choices is where the vendor says he is going to submit his own sales taxes to the Oklahoma Tax Commission.  If it is checked then a box is supposed to appear where the sales tax number can be entered.  I can't get it to work.  Attached is a screenshot.
