duplicates on form - cannot remove fields

On the application form https://www.jotform.com/build/61186865560160 - when you select the option for one child the Shoe Size Category field is repeated 4 more times. Can you fix this please?

I have inspected your forms and saw that several "Shoe Size category" fields are not associated with your conditions. That means it will always show whatever values the drop-down has. 

For example in this condition below:

You haven't added the Shoe Size Category in the selection. The same goes with your conditions for value 3-5. Please have that selected so only one will be displayed when the user chooses 1, only two if user chooses 2 and so on.
