Field Title missing in Configurable list

I have a problem in my form where the first Field Title missing in Configurable list. 

It might be caused by the CSS.

Don't know how to fix it.

I am sorry for the trouble caused to you. The issue was caused because of the error in injected custom css code. I have fixed this issue in your form and the first name is being now displayed correctly in the widget. Please check the screenshot below:

I would suggest you to please test your form again and get back to us if the issue persists or you need any other changes.

I am sorry but I remember it was a syntax error in the css code. It had additional ":" in the css code.
