    If I understood your requirement correctly, do you wish to use Custom Domain forms? If yes, you can follow this guide: How-to-Use-Custom-Domains-with-JotForm-Enterprise

    But do not that this feature is only available for JotForm Enterprise users. You will need to upgrade your account to an Enterprise plan to use a custom domain.
