Hi Nicanor!
Upon checking your account there is no reason for a suspension on your account however our Phishing Detector did flag your account as possible ClickBait.
I did go ahead and remove the suspension and you are able to have access to your forms. I do recommend registering your corporate email address to your account to avoid any further suspensions.

We need it back online and instead we get a link to this page in email after we gave them the information they need to remove the phishing suspension. I have to say I’m very disappointed in the customer service on this site.
We will not be using JotForm for anything else and will move to another service. We need this form to work as it was sent to all of our congregants and now there is a lot of confusion since the account is suspended.
