How do I open another form from within a form

    I have created a form for activities. But in case I have not put the activity down, I want to give the user an option to submit the activity.

    So, I'm using the single choice option, put in Submit an Activity or Program.

    Would like if they put a check mark in this option that other questions, pop up.

    If this is an option, can you tell me how to do it?

    I believe that you should use conditions. You should create additional questions and fields and hide them on the form. Then you should use conditions to display those fields, if your user checks the particular option.
