Please help me get access to my existing forms...

    I've tried to redo my password for my account at the email above.

    I know that at one point, I created another account because I was having trouble accessing the account linked to the form below.

    See also links to the survey on my website startupyourclass.ca

    Could you please guide me back to usage on this account, and another one if you can find it. My other email is ray.myrtle@gmail.com

    Thanks for the help. You forms work very well, I've just not been so active in the last couple of years.

    Could you please confirm, you are not able to login to the account rmyrtle? All your forms are located in this account only. If you try to reset the password, the reset link will be sent to the email address ray.myrtle@gmail.com.

    The account is active and in good standing. Please reset the password and try login to the account.
