If you want to remove the part other than the number in the form link, you can link your domain name in the form link and create custom domains. But this feature is available only for Enterprise subscribers.

Apart from this you can create custom URL names for your forms, but initial part of the form URL will be retained.

I understand that you are referring to the JotForm brand icon on the browser tab. Is that correct?

Sorry, but it cannot be directly removed from the Form Builder.

    But there is a workaround:

If you want to add your own favicon to your form, you need to download your form's full source code, then edit the HTML file and insert the code below between the <head></head> tags.

    <link rel='shortcut icon' target="_blank" href='http://www.domain.com/favicon.jpg' rel="nofollow" /><link rel='icon' target="_blank" href='http://www.domain.com/favicon.jpg' rel="nofollow" />

After that upload it to your own server then get the URL to publish the form.

Note that http://www.domian.com/favicon.jpg  is the URL where your favicon is stored.

    Hope this helps out.
