What does it cost to remove jot form logo

If you're referring to the JotForm banner below the form, thank you page and on email, you just have to upgrade to any of the paid plans like Bronze and they are automatically removed. The JotForm banner is only shown on the free/starter account. This is also indicated in the plans on the pricing page.
