Account suspended

Hi there,

my account has been suspended, i thought i had all fixes in place to adhere to the terms and conditions on Jotform. can you please let me know and i will fix any other items that require adjustment.

Your account got suspended because one of your forms is collecting passport numbers or sensitive information.

I checked your form and there's no Terms of use widget. Here's the link: https://www.jotform.com/widgets/terms-and-condition

Am I able to get back into my account so I can add the terms of use widget, I tried doing that and it was quite confusing.
