Monday integration is not working

my integration with monday.com was working fine until 2nd december. now it is not working and i tested it and it says internal error

I checked your form (91483737884473) and you have a phone and date & time fields in it.

Please refer to the related thread below for the workarounds/solution mentioned by one of our devs and see if that will fix the internal error on your form.
