Account suspended

    Hello

    My account has been suspended and I'm not sure why.


    Please could you unsuspend the account.

    I see that the account is auto-suspended by our anti-phishing system which is strict to scan each form to calculate the probability of phishing activities. It is not allowed to create login forms or collect sensitive information like Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on phishing.

    I have checked your forms and they are clean and normal. However, I have noticed that there are several fields without any labels on the form tilted 'Alternative Medicine-Patient Intake form' that triggers our system to check/suspend the account that you may need to take a look at. Please remove the unnecessary fields or add the questions/labels to these questions.

    I have activated your account and you should be able to access your forms normally. We are sorry for the trouble caused.
