How to create sections in form?

Hi I REALLY need assistance.

I need to create a section to our form for add-on's that also comes up in the total at the bottom of the form.

I also need for customers to be able to pick a day and a time slot that we allot for that day. 

Please help!

I checked your form and you seem to have managed to add page break for pages and a payment field that contains your products. You also have an Appointment Slots Widget which is the one I am going to recommend. I think is the best widget you can use for now.
