Why is my account suspended?

    Hi:

    I'm trying to see why my forms are offline.

    I have checked the forms in your account and it appears that the suspension was triggered by your Loan Application Form.

    The form was flagged by our system for suspicious phishing activity.

    Aside from that, your account also has a MasterCard Application Form that also has the MasterCard logo.

    Jotform implements scripts that allow us to detect forms that are collecting sensitive information, our terms are clear regarding such type of forms.

    Although we understand that this might not be the case with your form, we had to act according to the general rule to be in compliance with our Terms:

    "Free accounts with non-corporate emails showing suspicious forms must be suspended immediately."

    In order for us to reactivate your account, we will have to delete the forms I have indicated above.

    Or, you must provide us with the following information instead:

    1. Your website.

    2. Your corporate email address

    Please send an email to reviewers@jotform.com using your corporate email address. Include in the subject this support thread # 2127483 and briefly explain the purpose of the form.

    You can continue building similar forms in the future, but we encourage you to update the email address in your JotForm account with the corporate one to avoid the same inconvenience.

    I am ok with you deleting any form that ise considered to be outside of your terms and conditions.  Please do so and reinstate my account so that I can access my additional forms.
