My conditions are not working

Hi,

I wanted the rest of the form after Is the contract total $5,000 or greater? to show if yes is selected. I set up the conditions and it is not working. Please help.

I entered all of the conditions. Now that I checked, they were all gone. This has happened before and is very frustrating.

Could you please explain further what you're trying to achieve? 

If the YES option is selected, the below fields should show. Is that correct? 

If the NO option is selected, the below fields should hide. Is that correct?

The issue was resolved when I re-added the conditions. My problem is, that sometimes when I add conditions, like I did earlier today. They show in the system, then disappear like I never added them. This is the second or third time that this has happened.

Sorry for the inconvenience. We had an issue earlier about not saving form properly but our developers immediately fixed it. 

Could you please test it again and see if the issue still persists?
