    Hi @dcnorman!

I would like to apologize for the inconvenience you are experiencing, I am more than happy to assist you with your inquiry.

After some investigation, I have found that the form you are referring to is associated with an account that has been deleted. When an account is deleted, all forms from that account are disabled and the account is put on a 30-day hold before being completely purged from our databases along with all data stored in the account.

Since the account that is deleted is from the same domain as yours, I can re-instate the account so that you once again can access this form. We can also move the form to another account if you wish.

Please do get back to us so that we may continue to assist you, we look forward to your response!

As per your request, we have now moved the form to your account. Please note that your account is currently holding Free subscription plan and the moved form shall be displaying Jotform footer along with other limits of free plan. I see that there are 10 forms in the other account FOX24 and the paid subscription has been cancelled for the account.

Please check with the user (dlmiller@foxcharleston.com) of the other account if they have accidentally deleted the account. The user may request to reinstate the account using his email address.

    Thanks!
