Problem viewing form on ipad mini

Hello,

viewing my form www.q26.pl on iPad mini horizontaly, the form view is jumping from big to small, from small to big like crazy.
People can not read and feel it out. 
this is some technical problem. 
please solve it.

The form zooms out when you access the webpage on the iPad. Please add a meta tag for iPad to your website, here is a link with more information: https://www.w3schools.com/css/css_rwd_viewport.asp

That should resize the site and display the form properly.
